Renewal Dependent Pass

Dependent Pass Checklist

i – Applicant Passport Copy – All pages, must have more than 18 months validity and more than 7 blank pages.

ii – Passport Photos (2 Photos) – White background, dark attire clothes.

iii – Student’s Documents – Passport Biodata Page, Visa page, Offer letter, Verification letter from Faculty.

iv – Attestation from Embassy:

• Relationship Letter.
• Marriage certificate (original + translated copy).
• Children’s birth certificates (original + translated copy).

v – Bank Statement – Bank statement showing RM 30,000 final balance OR RM 10,000/month average for 3 months. Must be original and stamped by the bank.

vi – Medical Health Insurance – One (1) year medical insurance for dependents.

vii – School Verification Letter – For children aged 7+ (or a declaration if not in school).

viii – Admin Fee (RM40) – Pay online at payhub.utm.my and email the receipt.

IMPORTANT NOTES:

• All embassy letters and certificates need a current stamp.
• Children over 17 cannot be dependents.
• Your passport must have more than 7 empty pages.
